Call 360-252-9301 to schedule an appointment.  If you are scheduling a mammogram, you do not need a referral.  For all other exams, you will need a referral from your provider.  Please bring the order/referral form with you at the time of your exam.

REQUESTING MEDICAL RECORDS

Patients may request a copy of their medical records, Monday - Friday 8:00am-5:00pm.  South Sound Radiology requires 24 hours to process all requests for medical records.  If this is an urgent request, please contact 360-493-4646 or toll-free 1-800-624-9759 immediately so that we may attempt to accommodate your needs.

Patient medical records can be obtained in the following ways:

• Patients may visit South Sound Radiology in person with proof of identification.
• Fax (360) 493-4614 or mail the medical records request form.

*Note: Patient signature is required on the medical records request form. If picking up for a spouse, the Patient for whom the forms are being requested must still provide a signature.
